The Telephone Visits (2.16.840.1.113883.3.464.1003.101.12.1080) value set used in this measure includes CPT and SNOMEDCT codes that represent encounters for assessment, evaluation, and management services to a patient by telephone. According to the guidance in the CPT coding manual, codes for Telephone Services (98966-98968) are intended to be used for non-face-to-face assessment and management services for future encounters. Codes 99495 and 99496 are associated with Transitional Care Management (TCM) Services for patients who are making transitions from inpatient hospital settings to community settings. While depression screenings may be performed during TCM encounters, they do not meet the intent of the Telephone Visits value set or align with the intent of the CMS2v13 measure.
       
       The measurement team will continue to monitor the CPT codes and their potential impact on their measure and will consider inclusion in the value set in future years.

    • This would increase our performance for CMS 2

      We found that a PHQ9 screening for office visits (TCM 99495 and 99496) were not beingĀ Met for numerator performance. Upon investigation we found that TCM codes 99495 and 99496 were not included in the value sets of qualifying encounters for CMS 2 and that is why the patients were not meeting the measure. Can the 99495/99496 TCM be included as a Qualifying Encounter for CMS 2?
