      Thank you for inquiring about the CMS69v.12 Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan measure. To address your initial question, there are multiple ways to make it into the numerator:

      If the patient has a diagnosis of an abnormal BMI (whether a measured BMI number or documented diagnosis) and follow-up is documented (and directly associated with the abnormal result); or

      If the patient has a normal BMI.

       
      For intervention performed, since the operator “overlaps before” is used, the diagnosis must have been documented before the follow-up (and still be ongoing at the time of the follow-up intervention), and then the follow-up must also explicitly indicate that it was related to the abnormal BMI diagnosis.

      Can you please confirm that in order to meet the numerator criteria, is a diagnosis OR a reason required if the patient's BMI is above normal in additon to the follow up?

      Also, does the diagnosis have to be entered prior to the follow up being provided?
