       Thank you for your question related to CMS2v13. The measure does not require a specific score for any of the depression screening tools. Instead, the numerator uses SNOMEDCT codes to capture positive and negative depression screenings (428171000124102: Depression screening negative (finding)/428181000124104: Depression screening positive (finding)). Any screening tool-specific scores indicating a positive or negative depression screen can be mapped to either of these SNOMEDCT codes for the purposes of this measure.

      CMS-2 Preventative Care and Screening: Screening for Depression and Follow-Up Plan. We are seeking clarification related to the screening score/result. The specs state the depression screening does not require the documentation of a specific score, just whether the results are considered 'positive' or 'negative'. We are using the PHQ-9 screening tool which has criteria for what score constitutes a 'positive' or 'negative' result. Our EHR has a field that populates the PHQ-9 score in numeric value but need clarification if the addition of the score criteria to our documentation (based on the screening tool used) would be sufficient for the measure requirement. That score criteria basically says "No or minimal (0-4), Mild symptoms (5-9), Possible Major Depressive Disorder, Moderate (10-14), Moderately Severe (15-19), Severe (20+)".
