      Thank you for your questions about CMS1028v2 Severe Obstetric Complications.

      As stated in the human-readable measure specifications header for Risk Adjustment, "The risk variable definitions are included in the specifications and risk variable data should be sent with eCQM data in the QRDA1 file." The risk variable lab results for WBC and hematocrit as well as the risk variable physical exam results for HP and HR should be mapped so that all relevant data may be captured for the appropriate cases.

      The logic is looking for the first resulted value from 24 hours prior to the start of the encounter to before the time of delivery. If the EMR system is not able to access results from an outpatient visit or other hospitalization within 24 hours prior to the delivery encounter, then only results from the start of the delivery encounter to before the time of delivery will be evaluated for those four risk variables.

      2 questions:

      1. Is it mandatory to report all 4 Risk Variables for all patients?
      2. The Specifications mention, "Report the first resulted value 24 hours prior to start of encounter and before time of delivery". Can we report the first captured value for the encounter 24 hours BEFORE OR AFTER admission but BEFORE delivery? Suppose we had a patient whose labs and vitals were captured after admission, but before the delivery, is it acceptable to submit those values for the risk variables?
