Thank you for your question regarding CMS139v13.

According to the measure steward, the National Committee for Quality Assurance (NCQA), as stated in the measure header, a fall risk screening must assess "whether an individual has experienced a fall or problems with gait or balance. A specific screening tool is not required for this measure, however potential screening tools include the Morse Fall Scale and the timed Get-Up-And-Go test."

This is further specified in the measure logic and terminology. Fall risk screenings should be identified through codes in the “Falls Screening” value set (https://vsac.nlm.nih.gov/valueset/2.16.840.1.113883.3.464.1003.118.12.1028/expansion/eCQM%20Update%202024-05-02). Clinically equivalent services may be mapped to the codes in the value set. If mapping is done, you should maintain documentation in case of a CMS audit.

Please refer to the measure specification for measure denominator and numerator requirements: https://ecqi.healthit.gov/sites/default/files/ecqm/measures/CMS139v13.html. For guidance on how to read an eCQM specification, you may refer to resources on the eCQI Resource Center such as "Guide for Reading eCQMs": https://ecqi.healthit.gov/sites/default/files/2025-Guide-for-Reading-eCQMs.pdf.

    • By including the Hendricks Fall Assessment we would be able to increase our numerators and denominators

Our inpatient team uses Hendrick's Fall Assessment. Would we be able to use this fall screening tool and map it to CMS 139? Is there a list of assessments we can use to correctly map the measure?
