Thank you for your question. The steward has determined the logic should reflect a patient with a diagnostic study confirming VTE and who has received warfarin any time during the inpatient encounter, the patient will pass the denominator logic. The logic was revised accordingly during the April 2014 annual update.

Thank you for your question. The steward has determined the logic should reflect a patient with a diagnostic study confirming VTE and who has received warfarin any time during the inpatient encounter, the patient will pass the denominator logic. The logic was revised accordingly during the April 2014 annual update.

In the denominator logic for VTE3:

      OR:
AND: "Occurrence A of Encounter, Performed: Inpatient Encounter (admission datetime)" <= 1 hour(s) starts after end of "Occurrence A of Encounter, Performed: Emergency Department Visit (facility location departure datetime)"
      AND:
      AND:
OR: "Diagnostic Study, Result: VTE Diagnostic Test (result: 'VTE Confirmed')" <= 2 day(s) starts before start of "Occurrence A of Encounter, Performed: Emergency Department Visit (facility location arrival datetime)"
OR: "Diagnostic Study, Result: VTE Diagnostic Test (result: 'VTE Confirmed')" during "Occurrence A of Encounter, Performed: Emergency Department Visit"
OR: "Diagnostic Study, Result: VTE Diagnostic Test (result: 'VTE Confirmed')" during "Occurrence A of Encounter, Performed: Inpatient Encounter"
      AND:
OR: "Medication, Administered: Warfarin" starts during "Occurrence A of Encounter, Performed: Emergency Department Visit"
OR: "Medication, Administered: Warfarin" < 1 day(s) starts before start of "Occurrence A of Encounter, Performed: Emergency Department Visit"

If a patient comes in to the ED and has a diagnostic test while in the ED for VTE that comes back "VTE Confirmed", then is admitted and is given Warfarin for the first time as an inpatient, they would not pass the denominator logic for this measure.
The logic above would need to be modified to include warfarin administrations in the inpatient encounter.
