      Thank you for your inquiry regarding CMS69 (Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan). We apologize for the delayed response as this ticket was recently assigned to our project team. We can confirm that the patient’s BMI should be generated from a height and weight measured by the eligible clinician or their staff (including visiting or agency nurse staff) during the measurement period. Therefore, if the calculated BMI is from the reported measurement period and based on the visiting nurse’s measurements (i.e., not self-reported by the patient), then you may use that to report the measure. The height and weight does not have to be captured during the qualifying encounter. As per the guidance section in the measure specifications:

      “BMI Measurement Guidance:

      - Height and Weight - An eligible professional or their staff is required to measure both height and weight. Both height and weight must be measured during the measurement period. Self-reported values cannot be used.
      - The BMI may be documented in the medical record of the provider or in outside medical records obtained by the provider.”

      We are a small specialty practice and are going to be reporting on BMI screening and followup.  We use Visiting Nurses to follow up on our patients care.  Are we able to use BMI information that they provide us towards this measure.  Or does the BMI need to be calculated in the office.  We certainly will be managing the result just want clarification.
