      Thank you for your inquiry regarding the Appropriate Treatment for ST-Segment Elevation Myocardial Infarction (STEMI) Patients in the Emergency Department (ED) measure. Patient/family refusal is not included as an exclusion in the measure specifications. There are two reasons the measure developer did not add patient/family refusal as an exclusion. First, the measure developer received feedback that occurrence of patient/family refusal for an emergent condition, such as STEMI, is low. Second, capturing patient/family refusal is still an area of active standards development. This issue previously been brought to CMS’s attention and we will continue to monitor the feasibility for capturing patient/family refusal in future Annual Updates.

      Hello - for the CMS 996 measure, there doesn't appear to be an exclusion for patient refusal.  We have a patient who due to the patients age and status, the patient and family chose not to go forward with the PCI or the administration of fibrinolysis.  Wanted to put that scenario in for future measure development consideration.
