      Thank you for your inquiry for CMS124v12 (2024 Performance Period). According to the measure steward, the National Committee for Quality Assurance (NCQA), self-reporting is permitted given that the Cervical Cytology or HPV Test reports are documented in EHR, approved for entry by the provider, using any of the codes found in the value sets "Pap Test" (OID: 2.16.840.1.113883.3.464.1003.108.12.1017) or "HPV Test" (OID: 2.16.840.1.113883.3.464.1003.110.12.1059), have a non-null result, and are performed within the required timeframe. You will need documentation of these codes in structured fields of the EHR, as well as their date/time, for measure evaluation.

      CMS 124v12 - We are reviewing the exclusion criteria for this measure- can it be extracted only from structured data entered in the EHR or can patient reported information " ie I had a hysterectomy in 2000" also be used? 

       

      This was answered only partially. 

      Just to clarify because this was not clear in the response- do we need to report a Month + Date+ Year for patient reported procedures such as hysterectomy? or just year?
