      Clinically equivalent services may be mapped to the ICD-10, SNOMED, and other codes used in a measure's value sets to satisfy the measure requirement. We are unable to provide specific guidance related to the mapping of codes. We recommend you consult with your EHR vendor and clinical partners. You can also consult Quality Data Model, Version 5.6 to have a better understanding of QDM datatypes, attributes, and terminologies.

      This measure requires the present on admission (POA) indicator for risk adjustment. POA documentation is typically found in claims which correlates with ICD-10 and not documented within the EHR where the SNOMED correlation resides. We continue to monitor documentation/feasibility of POA and additional terminologies such as SNOMED for this measure.

      Hello JIRA,

      Referencing    CQM-7305 about mapping to discrete fields in the EHR.

      The answer you provided to the poster was this:

      Thank you for your question about CMS1028v2 andCMS1028v3: Severe Obstetric Complications. Responses to the question regarding homelessness would satisfy the Economic Housing Instability variable. Responses should be mapped in discrete fields (yes, no, or unknown) to the "Economic Housing Instability" value set (2.16.840.1.113762.1.4.1029.292).

      We too have not mapped to any discrete fields that ask the patient about their housing situation. (and have several opportunities in the course of the patient stay). 

       In reviewing the OID in the specs and the one you referred to - There are no SNOMED codes relating to anything (food, housing, etc) in the value set.  There are only ICD 10 codes.

      Please advise how to map discrete fields using ICD 10 codes.

      Please submit to the measure developer to address adding SNOMED codes to the value set so we can map to discrete fields in our EHR.
