      Thank you for your inquiry regarding CMS108 v12: Venous Thromboembolism Prophylaxis. From clinical perspective, patients won't be receiving Heparin IV push for general VTE prophylaxis. The provided example shows that patient also received 25,000 units after IV push dose. Therefore, this case would pass the numerator if 25,000 units IV continues infusion information is also pulled into QRDA file.

      Hello,

      Numerator "Encounter with VTE Prophylaxis Received From Day of Start of Hospitalization to Day After Admission or Procedure". 

      Day of Admission= 7/10/2024.

      Our QRDA file is pulling in Heparin 5,000 unit/mL 1mL inj 7/11/2024 08:27. Per the MAR and the Order the patient received Heparin IV Push 5,000 units =1 mL   7/11/2024 08:27 12,000 units.

      Per the MAR the patient received Heparin 25,000 units/ 250mL 0.45 NS RTU 25,000 units + premix 250mL 7/11/2024 11:39 IV Cont Infusion. 

       

      Is the QRDA file pulling in the Heparin IV Push correct or should the file pull in the Heparin IV Continuous Infusion?
