Thank you for your inquiry regarding CMS816v4, Hospital Harm – Severe Hypoglycemia. Humalog is the brand name for lispro, which is included in several RXNORM codes, found within the ‘Hypoglycemics Severe Hypoglycemia’ value set (OID: 2.16.840.1.113762.1.4.1196.393). To report on the measure, you must use the codes included in the applicable measure value sets.

Clinically equivalent services may be mapped to the codes used in the measure’s value sets to satisfy the measure requirements. While we are unable to provide specific guidance related to the mapping of codes, we recommend that you consult with your EHR vendor and clinical partners. If mapping is conducted, you should maintain documentation in case of a CMS audit.

Hello, we ran into a medication we want to clarify is acceptable.

Patient uses their own insulin pump labeled

'insulin lispro patient supplied pump bolus (HumaLOG)' in our system. However, I do not see anything that closely matches to this medication type in the RXNORM CODES given. Can you clarify that this is acceptable and if it is acceptable, are we then to just use anything with insulin rather than following the RXNORM CODE? Thank you
