eCQM STEMI Report Fallouts

      Thank you for your question regarding the CMS996v4: Appropriate Treatment for ST-Segment Elevation Myocardial Infarction (STEMI) Patients in the Emergency Department (ED). The intent of your question is to determine whether patients that are diagnosed with STEMI in the ED, but ultimately are found to not have a STEMI, are included in the measure calculation. In accordance with the measure specification, these cases are included in the denominator and fail the numerator since the PCI is not completed within 90 minutes. The issue of no STEMI found after a “cath lab” (exclusion of patients with no STEMI found after procedure) has previously been brought to CMS’s attention. The measure developer will consider further measure refinements to address this during an upcoming Annual Update.

    • Cases are being recorded as fallouts when they should not be in the patient population

      Multiple cases where patients come into the hospital through the ED and are diagnosed with STEMI in ED. However, when the patient is taken to Cath Lab and images are seen, it is revealed that the patient does not have a STEMI and there are no targets to PCI (Stent). These cases are resulting in fallouts on the eCQM STEMI report because PCI was not done within 90 minutes, when there is nothing to PCI. Is the report correct in considering these cases as fallouts?
