      Thank you for your inquiry regarding CMS816v3, Hospital Harm – Severe Hypoglycemia. RXNORM codes for Dulaglutide/Trulicity are included in the ‘Hypoglycemics Severe Hypoglycemia’ value set (OID: 2.16.840.1.113762.1.4.1196.393). These include:

      1551295: 0.5 ML dulaglutide 1.5 MG/ML Auto-Injector
      1551304: 0.5 ML dulaglutide 3 MG/ML Auto-Injector
      2395777: 0.5 ML dulaglutide 6 MG/ML Auto-Injector
      2395783: 0.5 ML dulaglutide 9 MG/ML Auto-Injector

      Please ensure that your client is using the appropriate codes. You can reference the Value Set Authority Center (VSAC) for additional details, including specific codes in this measure’s value sets (https://vsac.nlm.nih.gov/).

      Specific to your question about numerator compliance, could you kindly provide the referenced files, without PHI.

      Note that this ticket will be marked as resolved while we await additional documentation, however, you may respond to this inquiry via a follow-up comment.

      I have a question about how patients qualify for the numerator for HH-01. In the file we meet the requirements by the glucose result being less than 40 mg/dL, hypoglycemic medication administered within 24 hours prior to the start of the hypoglycemic event, and not repeat tests for glucose with a result of 80 mg/dL within 5 minutes of the 40 mg/dL result. Below is how we write the value is less than 40.

                        <value xsi:type="IVL_PQ">

                            <high inclusive="false" unit="mg/dL" value="40"/>

                        </value>

      We have a client telling us the patients are not meeting the numerator, and we are unable to test this in Cypress due to the IVL_PQ quantity. I can provide files without PHI, but any guidance on this would be helpful.
