      Thank you for your inquiry for CMS996v4: Appropriate Treatment for ST-Segment Elevation Myocardial Infarction (STEMI) Patients in the Emergency Department (ED).

      All patients that arrive to the ED with a STEMI diagnoses are included in the measure denominator, assuming none of the measure exclusion criteria are met. In addition, please note that discharging the patient from the ED to an acute setting within 45 minutes will count towards the numerator as one of the three possible numerator outcomes. In this numerator, a patient will need a disposition plan from the ED and may go on to an acute setting to receive diagnostic services, such as cardiac catherization or even cardiovascular surgery. This third numerator accounts for patients that may not go through the fibrinolytic or PCI pathway, for various medically indicated reasons.

      Would a pt be excluded if pt presents to the ED with dx STEMI.  Proceeds to catherization  lab and is dx with 3 vessel disease.  No PCI performed.  Consult for cardiovascular surgery.
