      Thank you for your inquiry regarding the Appropriate Treatment for ST-Segment Elevation Myocardial Infarction (STEMI) Patients in the Emergency Department (ED) measure.

      Patient/family refusal is not included as an exclusion in the measure specifications. There are two reasons the measure developer did not add patient/family refusal as an exclusion. First, the measure developer received feedback that occurrence of patient/family refusal for an emergent condition, such as STEMI, is low. Second, capturing patient/family refusal is still an area of active standards development. This issue previously been brought to CMS’s attention and we will continue to monitor the feasibility for capturing patient/family refusal in future Annual Updates.

      A patient arrives with a STEMI, however, the patient and/or representative makes a determination that end of life measures are appropriate and refuse thrombolytics, transfer & or Cath procedures.  Does the measure exclude these patients at any step in the flow and if not, what is the reason for excluding patient choice?  Thank you.
