      Thank you for your question on CMS71v13 and CMS71v14, Anticoagulation Therapy for Atrial Fibrillation/Flutter (STK-3). If there is a documented history, current finding, or diagnosis of ANY condition (e.g., remote, persistent, or paroxysmal) described as atrial fibrillation/flutter in the electronic health record, then the case should be included in the denominator. If there is no documentation of the applicable codes, found in the for atrial fibrillation/flutter within the "Atrial Fibrillation or Flutter" value set (2.16.840.1.113883.3.117.1.7.1.202), then the patient will not qualify for the denominator.

      Please reference the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/ to review value set codes. Click on the “Search Value Sets” tab and enter the value set ID to review codes included in the respective value set.

      We had a case where unclear h/o atrial fib

      I would just like a definitive answer if it should be included to bring back to our IT resource.
