      Thank you for your question regarding CMS108v12 (Venous Thromboembolism (VTE) Prophylaxis and CMS190v12 (Intensive Care Unit Venous Thromboembolism (VTE) Prophylaxis . The documentation of "Low VTE Risk" alone is sufficient, and when the patient was assessed within the qualifying timeframe, the case will be included in the numerator.

      One dose of pharmacological VTE prophylaxis administration within the time frame will make the case satisfy the numerator. The same is applicable for one mechanical VTE prophylaxis in use/applied.

      Hello,

      --Can you please tell me if documentation of "Low VTE Risk" alone, documented on a VTE risk assessment/order, is an acceptable reason for not administering either mechanical or pharmacological VTE prophylaxis? Does this pass the measure requirements or does the documentation need to include additional information, such as "Low VTE risk, no need for VTE prophylaxis"?

      --Does just one dose of pharmacological VTE prophylaxis within the timeframe pass even if, for some reason (medication DC/change, for example), there are no additional doses within the timeframe.

      --Does just one documentation/flowsheet entry of mechanical VTE prophylaxis in use/applied within the timeframe pass even if there are no additional uses within the timeframe or all other documentation includes statements such as "off - patient ambulating"?
