If SCDs were ordered and not documented as applied/administered on the day of or the day after hospital arrival, then two documented reasons are needed: (1) a documented reason that explicitly states why the ordered SCDs were not administered AND (2) a documented reason that explicitly states why pharmacological VTE prophylaxis was not administered.

    • Clarification on documentation to meet the measure

      Hello! Within our documentation system we have several anti-embolism intervention options to choose from to document VTE prophylaxis including: On, Medication, Refused, Amputee, Off (ambulating), Off (comment) and Not applicable for age group. If a patient has SCDs ordered (with no chemical VTE prophylaxis ordered and administered), would documentation of Medication or Off (ambulating) within the anti-embolism intervention flowsheet row meet the measure for SCDs or would we need to have documentation of On or Refused to meet the measure? Thank you!
