Alternatives to serum creatinine use in evaluating kidney function

      Thank you for your question regarding CMS832v2, Hospital Harm – Acute Kidney Injury. Currently, the measure only evaluates serum creatinine to measure kidney function.

      Within the ‘Rationale’ section of the header, specific to acute kidney injury diagnosis, serum creatinine levels are the “…most reliable and consistently available electronic data element…”. Additionally, as indicated in the ‘Clinical Recommendations’ section, serum creatinine is “…cited by many guidelines for defining and monitoring acute kidney injury”.

      Sir/Madam:

       

      The measure specified in the eCQM for hospital harms acute kidney injury is for serum creatinine to measure kidney function.  It does not mention alternatives to serum creatinine such as urine output or creatinine as measured in the urine in order to assess kidney function.

       

      Are these alternatives acceptable proxies for serum creatinine levels?
