Thank you for your inquiry. The measure evaluates whether there is evidence of someone living with diabetes receiving both eGFR and uACR during the measurement period. If someone with diabetes is receiving care in a health system with a common EHR that shares laboratory data among eligible clinicians, the measure should recognize that testing occurred during the performance period and give credit to all eligible clinicians that have access to that common laboratory data. If the care delivery system does not have a common EHR with shared access to laboratory findings, only the clinician ordering the testing will be recognized.
