Reporting MVPs as a group and not individual practitioners, does each practitioner need to submit documentation on each measure?
As a small practice, we currently have 2 physicians (lets call them Dr. A & Dr. B). We have a question regarding reporting on the measures that only need to be documented once within the measurement period. The way our clinic works is that most patients see the same MD until they are discharged from our clinic. For various reasons, patient's assigned to Dr. A, may need to see Dr. B. Dr. A has already documented on all of the measures we will report. Not including measures that need be documented at each visit (ex: CMS68, Documentation of Current Medications in the Medical Record), but measures that only need to be documented once during the measurement period (ex: CMS139, Falls, Screening for Future Risk), does Dr. B have to document on this measure even though Dr. A has already done so during the measurement period?
