      Thank you for your question. The STK eCQMs were developed to align with the chart abstracted measure criteria that TJC currently uses. The Discharge disposition value sets used for CMS71 and CMS104 are denominator exclusion criteria and are the same exclusion criteria included in the chart abstracted measures (CAM). Patients discharged to Home (prison/jail counts as home) and Other Healthcare Facility (SNF, LTACH) are included in the measure population.

      CMS72 is not looking at discharge but duration of stay < 2 days which excludes the case. Discharge Disposition is not used as exclusion criteria in the measure logic to calculate the reported rate.

      Hello,

      For chart abstracted measures for measure steward The Joint Commission STK2 and STK5 use for their DC Disposition as allowable values as Home, Hospice home, hospice healthcare facility, acute care facility, other health care facility, LAMA/, and expired.  However, in looking at the eCQM repository, I only can find acute care facility, LAMA, expired, and hospice.  I can't find discharge dispositions for home or the others.

      For the CMS steward, Inpatient chart abstracted Measures use as their Discharge Disposition for Other Health Care Facility:  Extended or intermediate Care Facility, LTACH, SNF?NH/VA Nursing Facility, Psych Hospital, Rehab Facility,  Transitional Care Unit, Veterans Home.

      What does the eCQM suggest we use when a patients are discharged to jail, home, SNF, LTACH, etc.?  You did not provide us with any value sets for these dispositions.

      Need Guidance as there is no value set for  jail, home, SNF, LTACH for DC Disposition
