      Thank you for your question regarding CMS506: Safe Use of Opioids - Concurrent Prescribing. As you noted, medications which count towards the numerator are prescribed during the inpatient encounter (OID 2.16.840.1.113883.3.666.5.307) which includes the following codes: 32485007 Hospital admission (procedure); 8715000 Hospital admission, elective (procedure); 183452005 Emergency hospital admission (procedure). In your example, if the opioids in the discharge documentation are during the three encounter codes, then they are counted towards the numerator. This could include the opioid prescribed in the ED if they are not discontinued.

      CMS506v6 - Need guidance on Numerator logic when one "Schedule II & III Opioid Medication" is prescribed during ED and then patient gets transferred to Inpatient and another Schedule II & III Opioid Medication is prescribed at discharge.
      The numerator intent says that the medication author datetime should be during InpatientEncounter, and because of that the patient will fall out of numerator.

      But since the prescription both ED and Inpatient are available after the patient gets discharge from inpatient department, shouldn't both med should be counted.
