      Thank you for your question regarding CMS506v6: Safe Use of Opioids – Concurrent Prescribing. The numerator for this measure includes "[i]npatient hospitalizations where the patient is prescribed or continuing to take two or more opioids or an opioid and benzodiazepine at discharge." If the second opioid is active on the medication discharge list, then the second opioid would count towards the numerator because the measure includes continuing opioids and/or benzodiazepines at discharge.       Our institution has an inpatient rehab (IP rehab), patient discharged to IP rehab with a discharge and direct admit order showed up in the numerator, the first opioid medication which the patient was receiving during his acute inpatient hospitalization was added to the discharge/readmit medication list to be continued after discharge. A second opioid medication that the patient was only receiving at home prior to his initial inpatient hospitalization was added to the "Ask your Doctor" section in the DC summary. Would the second opioid count towards the numerator, even if patient didn't have the medication ordered during his acute hospitalization or during his rehab stay . Thank you
