      The value set for STEMI should be revised to:
      1 - exclude use of ICD-10 codes for STEMI, since those codes are assigned at discharge, as opposed of during the ED encounter. In addition, those codes might be assigned to a STEMI diagnosis that occurred after the ED encounter,
      2 - exclude cases without a STEMI diagnosis on the first EKG in the ED.
      3 - exclude cases where the patient or family refused PCI.

      Incorrect cases are being included in measure denominator. Those cases include those without a STEMI diagnosis during the ED encounter. The measures is capturing cases with NSTEMI or with STEMI diagnosed after the ED encounter. Recommend revision of the value set for STEMI.

      As a result of inclusion of cases without STEMI in the ED, falsely low rates of compliance are being reported.  In addition, these cases pose an increased burden on our staff due to increased chart review required to discern true STEMI cases in the ED from others.
