      Thank you for your inquiry regarding CMS826v2, Hospital Harm – Pressure Injury. Per the measure description, this measure, for use in the 2025 reporting period, “…assesses the number of inpatient hospitalizations for patients aged 18 and older who suffer the harm of developing a new stage 2, stage 3, stage 4, deep tissue, or unstageable pressure injury.” In identifying new stage 2, 3, 4, or unstageable pressure injuries in patients, the measure’s numerator logic looks for codes in two value sets - "Pressure Injury Stage 2, 3, 4 or Unstageable" (2.16.840.1.113762.1.4.1147.113) and "Pressure Injury Stage 2, 3, 4, or Unstageable Diagnoses" (2.16.840.1.113762.1.4.1147.196). Please reference the National Library of Medicine’s Value Set Authority Center (https://vsac.nlm.nih.gov/) for more information on the value sets and codes included in the measure specification.

    • We are developing software to make monitoring of patient positioning and alerting to staff for the need to change position for patients at risk for pressure injury.

      I am reviewing the details for the Hospital Harm-Pressure injuries measure for the 2025 reporting period in the above link. Could you please confirm that new stage 2 pressure injuries will be included along with new stage 3, 4 and unstageable pressure injuries as a hospital acquired condition in 2025?

       

      I also read that reporting for this measure will be voluntary in 2025 and 2026 will be voluntary but will become required in 2027.  can you confirm?
