Version order for GMCS CMS986 specs

      Thank you for your inquiry regarding CMS986, the Global Malnutrition Composite Score (GMCS).

      A similar question about version numbers was addressed in a previous ticket (CQM-6944). To confirm, CMS986v4 with a patient age range of greater than or equal to 65 should be utilized for CY2025 reporting. CMS986v3 was crafted to support expansion of the measure population to adults 18+, recently included in the CMS final rule. The version (CMS986 v3 published for Reporting Period CY 2026) was built prior to the measure version developed for Reporting Period CY 2025 (CMS986 v4) to support the Measure Under Consideration (MUC) proposal process timeline.

      The measure was included on the program candidate list because of the substantive change in the measure (changing the initial population from greater than or equal to 65 years of age, to: “Inpatient hospitalizations during the measurement period with length of stay of 24 hours or more among individuals 18 years of age and older at the start of the inpatient encounter”). The updates to the measures’ ‘Initial Population’ are scheduled to begin in CY2026.

      In reviewing measure specs on the eCQI website we have 2 questions regarding the GMCS measure:

      1) The 2025 version of the measure is 4, but the 2026 version is 3. Is there a reason these aren't in chronological order?

      2) Just wondering why this is considered a program candidate eCQM when the eCQM is already a part of the annual measure list? Wouldn't these changes just be incorporated into the spec during the next AMU?
