Both the HOOS and HOOS JR are appropriate functional status assessments that may be counted towards the measure. Please see the appropriate codes below used in the measure specification. One of the types of FSA mentioned in your inquiry may be used towards the numerator requirement if performed within the appropriate timeframe and with a result. Please note that the same FSA instrument must be used for both the initial and follow-up assessment. This quality measure requires a functional status assessment via QDM datatype "Assessment, Performed" using the appropriate codes found in the measure logic, and also requires a non-null test result using QDM attribute "Result".       which one do we need to use, we have the patient fill out at their pre-op appointment and then 10 months later.
