Thank you for raising this issue. The National Kidney Foundation (NKF) is aware that in some laboratory information systems (LIS), when the level of albumin is below detectable limits in the urine, the LIS may respond by reporting “unable to calculate”. Unfortunately, a result that is non-detectable will not meet the numerator criteria for definition "Has Kidney Panel Performed During Measurement Period" at this time. The NKF will examine addressing this issue in the upcoming Annual Update.

The national laboratories and many large academic laboratories have developed workarounds for this issue. We have attached a document that provides suggestions for a workaround in the LIS if your institution conducts the laboratory processing. This document is directed to laboratory professionals. If this issue is associated with an external community laboratory, please provide the NKF with the name of that institution and we will contact them about the implementation of this workaround. The National Kidney Foundation has a team supporting laboratories who are interested in deploying this simple workaround.

    • Need clarification on how this scenario should be counted in the measure

For eCQM CMS 951: Kidney Health Evaluation, the numerator is “Patients who received a kidney health evaluation defined by an eGFR AND uACR within the measurement period”.

If the lab test for the Urine Albumin-Creatinine Ratio (uACR) within the measurement period cannot be calculated because the albumin level is low, does this count as a performed test for the measure? Would this patient (who has the eGFR performed within the measurement period) qualify for the numerator?
