      Question 1:

      Yes. Mapping SNOMED CT 410224008 (Mental health care education (procedure)) to documentation of the follow-up plan during the encounter or up to 2 days following the encounter would meet the numerator criteria for the follow-up plan. This and other qualifying interventions are included in the "Follow Up for Adolescent Depression (2.16.840.1.113883.3.526.3.1569)" and "Follow Up for Adult Depression (2.16.840.1.113883.3.526.3.1568)" Value Sets.

      Question 2:

      Yes. Referral to a primary care provider as a follow-up for a positive depression screen would meet the numerator criteria. This and other qualifying referrals are included in the "Referral for Adolescent Depression (2.16.840.1.113883.3.526.3.1570)"and the "Referral for Adult Depression (2.16.840.1.113883.3.526.3.1571)" Value Sets.

      These value sets are located on The National Library of Medicine's Value Set Authority

      Center (VSAC), https://vsac.nlm.nih.gov/ . The VSAC provides downloadable access to all official versions of value set content contained in the eCQM specifications.       I have two questions related to the Preventive Care & Screening: Screening for Depression & Follow-Up Plan measure:

       

      1. Will documentation of the following statement, on the same day of the positive depression screen, mapped to SNOMED CT 410224008 (Mental health care education (procedure)), meet numerator criteria for Screening for Depression and Follow-Up Plan, according to Performance Year 2025 specifications?                                                          “I educated the patient on the management of depression, including the role of physical exercise, talk therapy, and antidepressant medications."
      1. Does referral to a primary care provider by a specialist as a follow-up plan for a positive depression screen satisfy the numerator criteria?
