CMS-2: Does the exception guidance in CQM-7073 apply to both "Medical Reasons" and "Patient Declines?"

      Thank you for your inquiry into CMS2v12 Preventative Care and Screening: Screening for Depression and Follow-up Plan. Yes, the denominator exception "Medical or Patient Reason for Not Screening Adolescent for Depression" applies to both a medical reason for not screening as well as a patient reason for not screening. Therefore, if there is documentation that a patient declines screening it would count as a denominator exception and would apply to the entire measurement period. You do not need to document the denominator exception to every qualifying encounter after it was documented earlier in that measurement period. We hope this helps.

      In CQM-7073: (Do Exceptions apply for the full measurement year or encounter only?), the solution was, "Thank you for question on CMS2v13: Preventive Care and Screening: Screening for Depression and Follow-Up Plan. The denominator exception applies to the entire measurement period. You do not need to document the denominator exception to every qualifying encounter after it was documented earlier in that measurement period. ".

      Does this apply to both Denominator Exceptions of Medical Reasons and Patient Declines?
