For the hospice and palliative care exclusion, a consultation or referral to palliative or hospice care is sufficient to exclude the patient from the denominator, regardless of the outcome of the consultation. In your example, the case would be excluded.

      Hello,

      Can someone please clarify the term "receiving palliative" care. We have instances where patients are consulted by our palliative team, where a palliative consult order exsists. However, patients may still want aggressive treatment. In one particular case, the palliative team made recommendation on care i.e., consultative services, pain/symptom management. Would this case be excluded?
