Currently, we request that hospitals and EHR systems follow the measure specifications as they are detailed. We will consider revisions to the measures’ encounter diagnosis ranking approach in future updates.

      We'd like some direction on expectations for capturing diagnosis rank for the RAV as indicated in the logic for these new measures.

      Currently, when diagnoses are coded, the rank of 1 is captured based on the principal diagnosis. We are not aware of additional ranks applied to coded diagnosis.

      A quick review of several of the EHRs we support do not show additional rankings other than 1.

      Are you expecting hospitals to implement a new coding process to apply a rank of 2 and up to every coded diagnosis?
