How do we include range values for ejection fraction

      Thank you for reaching out with your question about CMS144v12 Heart Failure (HF): Beta-Blocker Therapy for Left Ventricular Systolic Dysfunction (LVSD). A range can satisfy the Ejection Fraction requirement so long as the observed range clearly meets the <=40% threshold noted in the denominator logic. To ensure the patient truly meets the denominator for the measure, the highest value in the range should be used to determine Ejection Fraction. For example, if a patient’s Ejection Fraction range is documented as 35%-45%, for the purposes of the measure, the Ejection Fraction should be noted as 45%, which mean the patient would not meet the denominator criteria.       The definition of LVEF < 40 % is described as : 'A range value should satisfy the logic requirement for 'Ejection Fraction' as long as the ranged observation value clearly meets the less than or equal to 40% threshold noted in the denominator logic. A range that is greater than 40% would not meet the measure requirement. "  Would a range that includes 40% be included in the denominator? For examples, how would we classify an EF range reported as  35-45% or 40-45%.. It simultaneously includes the limit 40% as well as ranges higher values.
