This question is in relation to CQM-6541. If using SNOMED CT 91310009 (Patient follow-up to return when and if necessary (procedure)) for self- referral to the same primary care physician (PCP) that completed the positive depression screen, along with additional follow-up plan in the medical record, does the additional follow-up plan need to be mapped to a structured field or value set in order to meet the numerator criteria? or can it be documented anywhere in the medical record but not in a discrete field?
