The RxNorm included in the value set of "Low Dose Unfractionated Heparin for VTE Prophylaxis" are the ones with strength of at least 5000 units/ dose AND administered subcutaneously. A heparin flush / small amount of heparin pushed to flush or maintain patency of a central line would NOT satisfying the criteria. Please reference value set "Low Dose Unfractionated Heparin for VTE Prophylaxis (OID: 2.16.840.1.113762.1.4.1045.39) for detailed RxNorm codes.     • low dose unfractionated heparin - order vs actual administration

      patients at a facility who are receiving Heparin IV push are being assigned an RXNorm code for "Low Dose Unfractionated Heparin" The order for the Heparin is originally for SQ administered, but the SQ order is being changed to IV, and administered IV push.  

      Please clarify if it is allowable that a SQ order for Heparin is assigned an RXNorm code for "low dose unfractionated heparin," regardless of the actual route of administration (IV).
