      Thank you for your inquiry regarding CMS122v12. Clinicians who have documentation of a qualifying encounter and an active diabetes diagnosis during the measurement period can report this measure. Therefore, a patient with an active diabetes diagnosis since 2021, with no end date, would meet the requirement for an active diabetes diagnosis.

      The 2024 version of QID 001 - Diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9%) states the Denominator includes: "Patients 18 - 75 years of age, with diabetes with a visit during the measurement period"

      We have interpreted this as the patient must have an active/open diagnosis of diabetes at the end of the measurement period. For example, the patient could have the diabetes diagnosis in 2021, and as long as there is no end date, the patient would qualify for that requirement.

      Our customer has interpreted this requirement as the patient needing documentation during the measurement period of the diabetes diagnosis.

      Please clarify the timing requirement for the diabetes diagnosis. Thank you
