An order for a consult for Palliative or Hospice Care would meet the requirements for the denominator exclusion, regardless of the outcome of the consultation.       There have been a couple of questions asked and answered that state that that the Consult Order would exclude the pt from the measure (CQM-4926 & CQM-5049), but the latest (CQM-6845) states, "... therefore having an order for a palliative care consult would not be included as a comfort measures intervention and would not qualify as a denominator exclusion for the measure.

      The value set does include a SNOMED code for a Referral to Palliative Care Services (306237005).

      Please clarify if an order for a Consult for Palliative or Hospice Care would meet the requirements for Denominator Exclusion.
