      Thank you for your inquiry about CMS108v12 Venous Thromboembolism Prophylaxis and CMS190v12 Intensive Care Unit Venous Thromboembolism Prophylaxis. These measures do not have a data element requiring the use of a specific risk assessment model or tool to determine VTE risk. If a physician assesses and documents that the patient is “low risk for VTE” within the qualifying timeline, then the case will satisfy the numerator condition.

      Caprini, Padua, and IMPROVE are externally validated models that may be used for VTE risk assessment. An internally developed VTE risk assessment tool may also be used. Please refer to your hospital VTE Prevention Committee, hospital-wide or unit-specific VTE prevention policies or protocols for VTE prophylaxis for further clinical guidance. We hope this helps.

      In the past TJC chart abstracted measures provided guidelines for collection of VTE Risk Assessment (i.e. Caprini, Padua, Improve) and Low Risk (i.e. Caprini of 0, Padua less than 4, IMPROVE of 0 or 1). Are there updated guidelines for what is acceptable for the electronic VTE measures? Are there additional screening tools that are acceptable? Can we use an internally developed instrument as long as it has been validated?
