Thank you for your inquiry about CMS125v12, Breast Cancer Screening.

It is correct that for the eCQM version of the measure, the age range is 52-74.

The PY2024 & PY 2025 Breast Cancer Screening eCQM does not include the lowered age range to align with the CQM version of the measure due to the delay in finalizing the draft USPSTF clinical recommendations. The final guidelines were not posted in the expected timeframe, therefore updates to the age range were not incorporated to the eCQM measure. We acknowledge the misalignment between the eCQM and CQM version of the measure. We anticipate incorporating the USPSTF guidelines which includes the lower age range of 40-74 in a future annual update cycle.

    • Discrepancies between the eCQM and CQM versions of CMS 125/Quality ID 112 are causing confusion for physician practices using this measure for MVPs and ACO reporting

For eCQM CMS 125: Breast Cancer Screening, the 2024 and recently published 2025 specifications on eCQI show the measure description to be “Percentage of women 50-74 years of age who had a mammogram to screen for breast cancer in the 27 months prior to the end of the Measurement Period” and the Initial Population is women 52-74 years of age with no stratification. CQM-6415 is a CRP from last fall (2023) that indicates the intent to change this measure to align with draft USPSTF guidelines to an initial population of 42-74 years of age with stratification “to permit continued trending for adults 50-74 years of age”.

The 2024 CQM version of this measure appears to have been updated with the expanded age range but the eCQM version continues to have the 52-74 age range. Will the eCQM measure be updated in 2024 and/or 2025 to follow the USPSTF guidelines or will the eCQM measure continue to have the 52-74 age range?

We also found this MVP page on the Quality Payment Program website that says the IP is 40-74 for all collection types. Can you clarify this discrepancy between the QPP/MVP documentation and the eCQI specifications?
