      Electronic Clinical Quality Measures (eCQM)’s rely on clinical concepts that are mapped to clinically equivalent data elements found in the measure's specified value set. You can reference the applicable codes containing the value sets on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/. Mapping is done within the Electronic Medical Record (EMR) to identify the specific data fields utilized by the eCQM. Unlike abstracted measures, eCQMs only pull data from structured fields, and cannot recognize data found in narrative charting. We recommend you consult with your EMR vendor and clinical partners to clarify this process. If unique mapping is conducted, you should maintain documentation of this process in case of a CMS audit.

      Are we to look thru the whole electronic chart for our validation on ECQM measures like we do with our abstracted measures or are we just to look at what our IT programmed? For example, PC02 the EGA is documented in multiple areas in the electronic record. If they are only using the H&P for the EGA and its not picking it up due to documentation, should we be looking in other areas that has the documentation?
