CMS 2 / CMS 138 -Does follow up plan need to be state specific?

      Thank you for your question regarding CMS138v12 (2024 Performance Period). Please note that the measure logic does not account for state-specific tobacco cessation interventions. The logic requires that interventions be performed within the required timeframe and documented using the appropriate codes.

      For CMS2v13: Preventive Care and Screening: Screening for Depression and Follow-Up Plan, the measure logic does not account for state-specific depression interventions. If the patient screened positive, a follow-up plan must be documented on the date of or up to two days after the date of the qualifying encounter using the appropriate codes in the following value sets:

      "Intervention, Order: Referral for Adolescent Depression" using "Referral for Adolescent Depression (2.16.840.1.113883.3.526.3.1570)"
      "Intervention, Order: Referral for Adult Depression" using "Referral for Adult Depression (2.16.840.1.113883.3.526.3.1571)"

    • Building Follow - up plan in Epic for a multi-state organization and would like to eliminate state specific language so it can be used by all EC's.

      We are updating our EHR build to include smarttexts for EC's to use to satisfy the Follow-Up specifications for CMS 2 / CMS 138 and want to confirm whether it needs to include state-specific depression or tobacco cessation interventions.
