Seeking assistance with QPP Multi Performance Rate Measure CMS 138 v12 "Preventive Care and Screening: Tobacco Use: Screening and Cessation Intervention”

      Reporting as an individual provider

I have a provider with initial patient population of 34, but only one patient in the denominator for measure #2.

If I  understand correctly, measure #1 is used for data completeness and we would meet that.

Measure #2 is used for performance rate for benchmarking.  Since we only have one patient in the denominator, would I be able to report this measure for this individual provider?

See attached screenshot of measure performances.
