      In your question it seems you are referring to the Denominator Exception not Denominator Exclusion (i.e., patient diagnosed with bipolar disease).

      In your example, if the patient has a screening in March (and met the Numerator), then returns for a visit in April and a documented patient reason for not screening is captured, the patient will still meet the Numerator (i.e., March encounter). The measure is patient-based and if the Numerator was met, it would report the Numerator for the patient.

    • Should patients with an early documented depression screening qualify for an exclusion if a patient or medical reason is documented at a later date?

      To qualify for a denominator exception the patient must have a documented medical or patient reason for not screening and not had a depression screening.   The has adult depression screening is looking at the entirety of the measurement period.  If a patient has a screening in March, then returns for a visit and April and a documented patient reason for screening is captured, does the patient qualify for the exclusion?
