      Thank you for your follow up question on CMS819v2, Hospital Harm - Opioid-Related Adverse Events.

      The measure captures opioids administered in the operating room if the operating room/suite is during the inpatient encounter. In your example, the operating room/suite is during the ED encounter. The patient would meet the Initial Population/Denominator definition because the logic does not recognize the operating room/suite due to timing (prior to the inpatient encounter), but it does capture the opioid administered during the ED timeframe. We will consider making updates to this logic during the next eCQM Annual Update.

      The 'Encounter with Opioid Administration Outside of Operating Room' definition includes the evaluation of: 

      • Qualifying Encounter
      • Opioid Administration
      • Inpatient Encounter facility location (Operating Room)

      The Timing for the Opioid administration starts during Global.HospitalizationWithObservation (this includes qualifying ED or Observation timeframe)
      However, the Operating Room.Suite location period is limited to the 'Inpatient Encounter'
      and not exists ( InpatientEncounter.facilityLocations EncounterLocation
                where EncounterLocation.code ~ "Operating Room/Suite"
                  and Global."NormalizeInterval" ( OpioidGiven.relevantDatetime, OpioidGiven.relevantPeriod ) starts during EncounterLocation.locationPeriod

      Is the measure's intent to qualify the following scenario for the measure's Initial Population?

      Emergency Department Visit 5/1 0800 - 5/2 0900
      Operating Room location period 5/2 05:00 - 5/2 0600
      Opioid Administration 5/2 05:30
      Inpatient Encounter 5/2 09:01 - 5/7 18:00

      The Opioid was administered during 'HospitalizationWithObservation'. The Opioid administration was during an Operating Room (OR) location timeframe. However, the OR times are not during the Inpatient Encounter timeframe.
