CMS 506 - Ativan IM was captured from interfacility transfer order but patient was discharged to home without any Benzos.

      Thank you for your question regarding CMS506v6: Safe Use of Opioids - Concurrent Prescribing. The initial population is defined as "Inpatient hospitalizations (inpatient stay less than or equal to 120 days) that end during the measurement period, where the patient is 18 years of age and older at the start of the encounter and prescribed one or more new or continuing opioid or benzodiazepine at discharge." Therefore, if Ativan was not listed as continuing at discharge and there were no other opioids or benzodiazepines listed, the patient should not be included in the initial population.

      QRDA captured Ativan IM from interfacility transfer orders. However, patient was discharged to home and no Ativan was ordered to go home with on AVS or discharge summary. Is this mapping still correct although the patient was discharged to home, not another facility?
