eCQM 68 measure question

      Thank you for your inquiry regarding CMS68v13: Documentation of Current Medications in the Medical Record. This measure is an episode-based measure and NOT a patient-based measure. The measure requires attestation of documentations of medications on the same day as the qualifying encounter to meet the numerator criteria. Each qualifying encounter in the measurement period requires a separate medication review and attestation that the review was completed. In the example you provided, the encounter on 4/8/24 should be included in the numerator, but the encounter on 4/19/24 without documented medication review should NOT be included in the numerator. Documentation or review of patient allergies is not required for this measure.

      Hello, We have a question regarding Documentation of Current Medications in the Medical Record. Please clarify this statement: "This measure requires documentation for all visits occurring during the measurement period for patients aged 18 years and older." For "all visits" -does that mean all UNIQUE visits, or each time the SAME patient comes in throughout the year? It reads as if it's every time a patient has an encounter w/in the reporting period, even if the measure was already satisfied for a particular patient. If that same patient comes in again, do we have to reconcile again? We are asking because I notice that if a particular patient was here on 4/8/24 and met the numerator, and then came back on 4/19 and we did NOT review medications, in our EHR report, he still meets the numerator. Also, are documenting/reviewing allergies still part of this measure or is it just the medications? This is affecting us because I am not sure if the report is being calculated correctly.

      I can understand if the data needs to be entered for the same patient every time that patient comes in during the reporting period, but if they came in 3 times already, and for one visit, we didn't log meds, shouldn't that patient NOT meet the numerator any longer?

       

      Also, we need to know if allergies reconciliation is part of the measure since it is not mentioned anywhere in the specs
