Thank you for your inquiry specific to CMS816v3: Hospital Harm - Severe Hypoglycemia and CMS871v3: Hospital Harm - Severe Hyperglycemia.

The LOINC codes in the value set referenced in your question (Glucose Lab Test Mass Per Volume, 2.16.840.1.113762.1.4.1248.34) represent glucose laboratory tests and indicate that results from those glucose laboratory tests should be reported as mass/volume. You can reference the applicable codes containing these value sets on the Value Set Authority Center (VSAC) at https://vsac.nlm.nih.gov/. Click on the “Search Value Sets” tab and enter the value set ID to review codes included in the respective value set. The LOINC codes within this value set do not specify a unit of measurement (e.g., mmol/L or mg/dL) to use for the mass/volume results. However, as stated in your question, the ‘Exclusion Criteria’ section of the value set’s metadata states that the value set “excludes concepts that represent a laboratory test measuring glucose and reported as Moles Per Volume (mmol/L).” The measure uses specifications that can be found at the eCQI Resource Center https://ecqi.healthit.gov/ specifically: ["Laboratory Test, Performed": "Glucose Lab Test Mass Per Volume"]

For Hospital Harm HH-Hypo and HH-Hyper, if Glucose Lab Test results are found in the EHR with mmol/L as unit of measure, should they be evaluated after converting them to mg/mL?

Or, should only results with mg/mL as UOM be evaluated?
