CMS68v13: Should the Numerator count newly prescribed meds as part of med reconciliation

      Thank you for your inquiry regarding CMS68v12: Documentation of Current Medications in the Medical Record. The measure does not currently assess if a patient has been newly prescribed medications in the qualifying encounter when determining inclusion in the measure numerator. As stated in the measure specifications’ numerator description, qualifying encounters where the eligible clinician attests to documenting, updating, or reviewing the patient's current medications are included in the measure numerator. Scenario 1 should be included in the numerator as medication reconciliation was performed. Based on the information provided, Scenarios 2, 3, and 4 would not qualify for numerator inclusion as medication reconciliation was not performed. We hope this helps.

      Can you please confirm if newly prescribed medications on the day of the encounter in the IP/Denominator should be included in the Numerator for the following patient scenarios:

       
      Scenario 1:
      ED patient is transferred to Inpatient status, Med Reconciliation is performed during the transfer. Should this count in the Numerator?
       
      Scenario 2:
      INP has no known medications, new medications are entered at discharge. Should this count?
       
      Scenario 3:
      INP has home medications which are confirmed on their Meds list, no new meds are prescribed at discharge. Should this count?
       
      Scenairo 4:
      INP has home medications, new medications are prescribed at discharge. Should this count?
